  Injectable Fillers

What is the source of the material (natural or synthetic)?

All sorts of materials can be used. The most popular material so far and the one that we use is synthetically derived, stabilised Hyaluronic Acid (HA).

How long has it been on the market?

Injectable bovine collagen was first introduced in 1997 and synthetically derived HA in 1996.

What kinds of clinical studies have been done?

Numerous studies have been done to determine the safety and efficacy of collagen and HA for the treatment of wrinkles. Other fillers that have more recently come onto the market (e.g. the permanent fillers) are still awaiting proper evaluation.

What are the possible side effects?

These are mostly injection related reactions such as swelling, redness, pain, itching, discolouration and tenderness at the implant site. These typically resolve within a few days after injection into the skin and within a week after injection into the lips.

Is skin testing required?

For collagen you will need a skin test which if equivocal may have to be repeated before treatment is started. For synthetic fillers skin tests are not normally required but in rare circumstances your doctor may advise you to have a test if they think it is appropriate.

Can I be allergic to it?

1 in 2000 treated patients has experienced localised reactions thought to be of an allergic nature.

What does a reaction look like and how long does it last?

A reaction normally consists of swelling at the implant site, sometimes affecting the surrounding tissues. Redness, tenderness, temporary visible lumpiness and on rare occasions acne-like formations have also been reported. These reactions have either started a few days after the injection or after a delay of two to four weeks, and have generally been described as mild to moderate and self-limiting with an average duration of two weeks.

What can be done if I have a reaction to it?

You should discuss this with the doctor who performed the procedure and they will advise you on any further treatment if necessary.

How long will the filler last?

How long a treatment holds its effect is very individual. It depends on many factors, such as your age, skin type, lifestyle and muscle activity as well as on the injection technique. Collagen is the shortest lasting filler (3-4 months). Hyaluronic acid based fillers last longer, 6-12 months if injected in the skin and 6 months for lips.

How many treatments will I need and how often?

This is best determined at a consultation. You may need a top up treatment and your doctor should be able to advise you on that.

What are the risks?

Like any such procedure the injection of fillers is associated with an inherent risk of infection. The use of specifically formulated aftercare lotion will reduce this risk to a minimum. There is a potential risk with the procedure that the material could inadvertently be injected into a dermal blood vessel, which could lead to vascular occlusion and corresponding consequences. No such cases have been reported for Restylane®, Perlane® and Juvéderm® to date.
